This form allows you to upload selected images and to write accompanying words to create a  single or double-sided page. Upload these through-out the year when you have a special event or story to tell or after our on-line session. 

Please upload or send us the original image or largest version of your image (this should be at least 300dpi, which is usually 2MBs or more). Our site accepts uploads up to 4MB per image. You can also email a link to our email if that is preferable.

Steps

1. Select 1-4 images for both the front and back of your page
2. Add a page title and captions for images (optional)
3. Write a paragraph or two capturing your story.

All fields are optional, so if you would like only one image on a page, then upload image 1 only. Then go to the back of the page and repeat with your next image(s).

If you are ordering multiple display pages, complete this form and submit for each individual page. Your uploaded pages or images can be see under the menu item My Story Uploads.

We will email you a no obligation proof of your display page(s) before printing and delivering them to you.
